There is a non-refundable fee of $245 for this service. This form allows you to order license information in certain settings, including: A verified certificate is a certified license history that covers a specific period of time. The verified certificate is mainly used for legal proceedings involving contractors and is admissible in court as prima facie evidence of the facts stated on the certificate. The document covers the last three years of license status, unless otherwise requested. The verified certificate includes the name and address of the contractor`s business, the business classifications in possession, the date of issue of the licence, the licence personnel, the information on the bond (if requested), any enforcement action taken by the licence and the status of the licence during the period covered by the certificate. The fee for each verified certificate is $67. Wait about six to eight weeks for a response. N/A = Qualifier is not a person applying for a single ownership license Asbestos certification and C-22 classification are not only required for tendering purposes as long as the contractor completes a subcontractor and the work is performed by a duly licensed asbestos remediation company. Use this form if you are a licensed contractor currently performing asbestos-related contracts in relation to the licensee`s other classifications. If a licensee has qualified for this certification, it is added to the license in the same way as an additional classification.
To obtain asbestos certification, a contractor must be registered with the Occupational Safety and Health Division (Cal/OSHA). This step will give you information about what you need to submit to get your license. (Turn on “No” if the license or registration has been suspended due to the lack of coverage, workers` compensation or other required insurance, qualifier, or family support.) A currently renewed but inactive licence can be returned to active status by submitting the “Application for Reactivation of an Inactive State Contractor Licence”. Some restrictions apply. these are explained on the application. If you consider your license to be “inactive” but has actually expired and the expiration date is within five years, order a renewal application. If your licence expired more than five years ago, you must submit an original application and request the reassignment of the licence number. Applicants who have been criminally convicted are not automatically refused admission – each application is examined individually on the basis of the applicable legal sections. When reviewing criminal convictions, the CSLB considers factors such as the nature and seriousness of the offences, the time elapsed since convictions, and any evidence of rehabilitation provided by the applicant. The CSLB contains several provisions for the review of criminal convictions, including Title 16 of Sections 868, 868.1, and 869 of the California Code of Regulations. Section 868 describes how to assess whether a criminal offence is essentially related to the qualifications, duties or duties of a licensee or registrant.
Section 868.1 determines whether a financial offence is directly and negatively related to the qualifications, duties or fiduciary duties of a licensee or registrant; and section 869 specifies whether an applicant has demonstrated rehabilitation. After applying in this package, you will find the Professional Experience Certification Form, which must be completed by the qualified person on application and certified by a qualified and responsible person who has direct knowledge of the listed experience and the specified period. The experience must be verifiable by payroll or similar documents. Corrections to certification forms must be initialled by the certifier. Forms that contain strikethroughs or amendments that could affect the validity of the Certificate of Professional Experience may not be accepted. You must complete the personnel information requested in sections 3 and 4 of the application for each person and company listed in the license. Your filing fee number, also known as the “receipt number,” will also appear on the front of your cancelled cheque, on the notice of appearance under review, and on an application to file the appropriate documents and initial licence fees. Please keep this number and use it in all applications regarding your pending application. A Qualified Person Bond (BQI) is required in addition to the Contractor`s Bond if the qualifier is a Responsible Agent (TRA). .
